If your scholar has asthma and/or will need an inhaler at school, complete and return the following to the front office.  Note, one or more of these forms need to be completed by a licensed physician.

• Asthma Action Form (English / Spanish)
• Medication Authorization Form (English / Spanish)

If your scholar has food allergies (including milk) or other allergies, complete and return the following to the front office.  Note, one or more of these forms need to be completed by a licensed physician.

• Medical Statement for Students with Unique Mealtime Needs (English / Spanish)
• Fluid Milk Substitution Form (English / Spanish)
• Anaphylaxis Emergency Action Plan (English / Spanish)

If your scholar will need medication (prescription or over-the-counter) at school, complete and return the following to the front office.  Note, one or more of these forms need to be completed by a licensed physician.
